Your Clinic Visit

Michigan Avenue Immediate Care is conveniently located in the Loop of downtown Chicago. Our medical providers strive to provide extraordinary quality care and service and believe it is a privilege to serve Chicago’s community including schools and their students, businesses and their employees, those living downtown, and hotel concierge and guests of the city.  Michigan Avenue Immediate Care is a walk-in clinic and no appointment is necessary.

 

If the need for non-emergent care arises and you would like to see a doctor, we welcome you to visit our clinic.

We understand that a visit to the doctor is not your first choice when deciding to do with your day. In addition to providing high-quality care in a convenient location, our physicians and staff try to simplify the registration process and make every effort to minimize your wait.

What to Expect

 

After you arrive at our facility, you will:

  1. Check in with the front desk to receive your registration paperwork.  Our staff member will request your driver’s license or other government-issued i.d., and your insurance card.

  2. Review and complete the following forms (Online registration coming soon):

    MAIC New Patient Registration
    MAIC Administrative Policies

    Records Release (If applicable)

  1. Return your registration forms to our staff member.

  2. You will be seen by a doctor in the order in which you arrive.

  3. After you have been seen by the doctor, and have received any other tests, vaccines or medical services, you will check out with a member of our staff.  At this time, you will be responsible for payment of the services rendered at today’s visit.

* Please note: Patients are seen in the order of arrival.  If you decide to complete your registration forms online and submit them prior to your arrival, this will not affect where you are in the queue of patients to be seen.


Return Patients

Welcome back! 

 

As a return patient, we will ask you for your identification and if anything from your previous registration has changed (i.e. have you moved? Do you have different insurance?, etc.).

Additionally, we will accept any other PPO plan not on the above list.  However, if we are not in-network with the PPO (not one of the companies listed above), the patient may be responsible for a larger portion than if they see a physician that is in their network.

 

Insurance Plans That Will Not Cover Visits In Our Office
Unfortunately, we can’t bill HMOs that we are not in network with (e.g. BCBS HMO).  If we are not in network, the insurance company will not cover the cost of the visit.  To help make the cost of the visit affordable for our patients, we do offer a self-pay discount.

 

If you have specific questions about your coverage please contact your insurance company. We suggest contacting your insurance company’s customer service department by calling the number located on your card to help you locate a provider.
